After investigating many products, we decided upon Prezza's Ultimate Survey.
It is reasonably priced, extremely flexible and easy to use. We like the
fact that it's hosted on our own server, and allows our authorized
departments and committees to easily create and maintain their own surveys,
including result graphs and/or CSV downloads. All this is done from any web
browser on any platform.
With the Professional version (not "Enterprise" / .NET), you also get the
full source code that you can further modify to fit your needs. (We changed
some of the layout and formatting)
Since we launched this software last December, we've been stunned at the
number of users, departments and committees that are now using it, and more
are asking for it all the time.
All it takes is setting them up with an account, and about a half hour or so
of IT staff time to train the user. After they create a survey, the user
generally gets so excited about it that they tell others...
